At night the home has two Registered Nurses assisted by four carers. Crawford unit on the ground floor is devoted to caring for people who have mainly physical nursing needs. Founded in 1987, At Home Care is a not-for-profit Article 36 Medicare-certified home health agency in partnership with Bassett Healthcare Network. AHC provides in home professional and paraprofessional services in Herkimer, Delaware, Otsego, Chenango, and Schoharie Counties. We also provide consumers information on twelve-step fellowships to support the treatment process as well as providing them information on meetings in their local communities. Our program provides a structured framework that is rooted in both motivational interviewing as well as cognitive behavioral therapy.

We have a community matron who visits weekly and we have a very good relationship with her. Ratings are provided by Care Quality Commission and reflects the most recent report for this care home, which was published on 17 October 2018. We provide good training for our staff and work to ensure they have the appropriate skill mix to meet the requirements of those who make New Bassett House their home.
AMS Care, providing premier home care for the elderly
All our residential, short break and short term home care services are regularly inspected by the Care Quality Commission to ensure they comply with national minimum standards. Hyde and Wren units are for people with dementia nursing requirements. At Home Care participates with most health plans within our 5-County Region, including Medicare and NYS Medicaid. Sliding scale and privately supported home programs are available. At Home Care, Inc. is accredited by the Joint Commission for demonstrating continuous compliance with its performance standards.
We are in close contact with the Salvation Army & have attended the services they hold every Friday in the village. New Bassett House doesn’t have a regular church service but the local Revenend Barbara Dale will come in to hold a service if we ask her. We have large bathrooms and toilets where staff, a service user & a wheelchair can move easily. We reviewed the information and data available to us about Bassett House on 8 December 2022. We have not found evidence that we need to reassess the rating at this stage. We have recently been involved in a video about Dementia support.
Inspection ratings
Youth are actively involved in the goal setting process that leads to the creation of their treatment goals. We have therefore encouraged every provider to open their ‘online windows and doors’ to help you make an informed choice. The profile score is a measure of how much information has been provided. The property is set near the town centre which is a 5 minute walk away from Shirebrook, close to the market place where there is a selection of shops. There are gardens all around the home with seating areas for people to sit in. They inspect care services to ensure they are supporting peoples wellbeing; appropriately setup; properly planning out care; are well led; and effectively staffed, each criteria is given a grade out of 6. Bassett House is a purpose built home, which caters for sixty-three residents with dementia and elderly people who require nursing care.
Care home group
We get involved the local schools & they come to us to sing at Christmas. If anyone wishes we can contact the local church or chapel 7 they will come in and hold a service. The salvation army are in the village and they come up often to sing. We have a medical room which can be used by the visiting district nurse.
Bassett House will keep referral agencies involved while the consumer is in treatment. The youth's progress is reviewed with the referral sources on a consistent basis; through phone communication with the youth's counselor and case manager. Our staff is trained in the TACT-2 Professional Crisis Intervention Training in order to create a safe and therapeutic environment for our youth. We provide a regular programme of social activities, hobbies and leisure interests. This includes the choice and opportunity for people to participate as fully as possible in local community activities. The home cares for adults of all ages with physical disabilities. Additionally, Rockley Core Unit is able to support people with learning disabilities and mental health conditions.
The Home is divided into three units to care for those requiring dementia or frail elderly nursing care. Bassett House takes this responsibility of networking and referral very seriously. Each aftercare plan is designed with the input from the client, parent and referral agent. The twelve step fellowships , as well as community based programs and services, are major components of these plans. Groups that the adolescents attend daily cover many different areas that will provide them with a wide variety of skills needed to improve their ability to be successful in their recovery environment. We strive to help connect the youth with their families as well as other support systems in their recovery environment.
The grounds to Gifford House extend around the Home and incorporate a large area laid to lawn, landscaped flower borders with various shrubs and rose arches. There are concrete paths leading around the property, several sections of which are wheelchair accessible. The rear exit doors to the Home lead out onto a large patio with tables and chairs where barbecues are often held. All of our rooms are fully furnished and have an en-suite facility equipped with a Nurse Call System.
